This course is to be taken by individuals who DO NOT yet have a license in the state of Tennessee and are going to or already have applied for one.

In order to register for the class please follow the instructions below.

1.) Please mail, 2416 Music Valley Drive, Suite 119, Nashville, TN 37214 a check or money order, made payable to Maj-lis Nash in the amount of $125.00

· No Credit cards are accepted.

· You may also stop by and drop off cash as a payment

· Please write the date you want to attend on the check.

2.) Please fax (615-360-8552) or email ASAP a copy of your massage school transcript so we may verify as fast as possible your transcript matches TN requirements.

· Include your name and phone number as well as email address on the fax

3.) Please bring the following items with you to assist us in assisting you

· A copy of your birth certificate OR your passport if it is current so that we may make copies and notarize them here for you.

· Any and all paper correspondences with the TN massage board

· Print out the attachments, fill out as much as you can and bring with you.

The course will consist of familiarizing you with the laws and rules in Tennessee regarding massage therapy, assisting in the completion of all paperwork necessary to apply for your license, submitting a transcript of attendance in this class directly to the board for you and getting you on your way to being licensed in the state of Tennessee.

All classes will be held on site from 9am-2pm.

Ethics, A&P and other courses will be individually scheduled and priced per hour needed.

You will receive all the necessary application papers to apply for both your TN state license and/or MBLEx exam if necessary.  In-House Notary is available to notarize your applications and any other necessary documents.
